APPLICANT REQUESTS THAT:

His Fitness Assessment (FA), dated 27 Oct 2011, be removed from 
the Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT:

He was injured during the FA in question.

In support of his request, the applicant provides copies of his 
AF IMT 348, Line of Duty Determination (LOD); Individual Fitness 
Assessment History, memorandum from the State Personnel 
Superintendent, and various other documents associated with his 
request. 

His complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

On 28 Oct 2011, an LOD was initiated stating “Injured while 
doing sit-ups.”

On 2 Nov 2013, the Fitness Assessment Appeals Board (FAAB) 
disapproved the applicant’s request to remove his 27 Oct 
2011 FA, from the AFFMS.  The basis for their decision was that 
the documentation provided does not support timely commander’s 
invalidation.

The remaining relevant facts pertaining to this application are 
contained in the letter prepared by the appropriate office of 
the Air Force.  Accordingly, there is no need to recite these 
facts in this Record of Proceedings.

THE AIR FORCE EVALUATION:

AFPC/DPSIM recommends denial.  DPSIM states that in accordance 
with AFI 36-2905, Air Force Fitness Program, "If an Airman 
becomes injured or ill during the FA and is unable to complete 
all required components, he/she will have the option of being 
evaluated at the Medical Treatment Facility but his/her test 
will still count unless rendered invalid by the Unit Commander."  
In this case, the applicant was able to complete the medical 
evaluation; however, did not provide documentation from the Unit 
Commander indicating his/her decision to invalidate the FA.   
Paragraph 13 also states that, "The Airman should notify his/her 
Commander in a timely manner regarding the injury/illness to 
ensure communication regarding test validity with the Military 
Treatment Facility and Fitness Assessment Cell staff occurs 
prior to score entry into AFFMS," which the applicant failed to 
accomplish.  Furthermore, the LOD provided by the applicant does 
not provide any information that includes diagnosis, prognosis 
period and type of physical limitations or restrictions, and 
does not replace the AF Form 422, Physical Profile Serial 
Report.

APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION:

In further support of his request the applicant provides copies 
of LODs and other documents associated with his appeal.  

His complete response, with attachments, is at Exhibit D.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
relief.  We note DPSIM recommends denial stating the applicant 
has not provided validation from his commander to remove the FA 
in question.  However, he has provided medical documents that 
reflect he aggravated a previous LOD injury while performing the 
sit-ups component of the FA.  He also provided a witness 
statement from a certified registered nurse that corroborates 
his injury.  Therefore, in view of the above, we find the 
evidence presented is sufficient to recommend the applicant’s 
records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air 
Force relating to APPLICANT, be corrected to show that his FA 
dated 27 Oct 2011, be declared void and removed from the AFFMS.
